10/23/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Broadnax indicated her day was well. Ms. Broadnax was well groomed and prepared for the session. Intervention: QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings, and emotions. QMHPA encouraged Ms. Broadnax to identify her present feelings. QMHPA supports identifying triggers of anger and depression. QMHPA discussions to help Ms. Broadnax with associating how stress can lead to depression, anxiety, and health problems. QMHPA asked Ms. Broadnax, if being stressed has ever caused her to make irrational decisions. If so, why? QMHPA asked Ms. Broadnax what were some alternate ways to dealing with stress that are stress relievers. QMHPA provided supportive listening. QMHPA encourages using positive communication to relieve stress. Ms. Broadnax was encouraged to participate in a relaxing activity. These activities were walking around the neighborhood and listening to music. QMHPA continued to encourage Ms. Broadnax to identify triggers of anger and depression. QMHPA engaged Ms. Broadnax in discussion about the cause of her anxiety and depression. QMHPA used active listening as Ms. Broadnax communicated. Response: Ms. Broadnax appeared to be receptive to QMHPA’s interventions and services that were provided to her. Ms. Broadnax communicated to QMHPA she was challenged this week with identifying ways to communicate her needs and the needs of her family effectively. Ms. Broadnax shared that she understands how important it is to have a positive image about yourself but this week she felt she was not confident about herself or her actions. Ms. Broadnax shared that she felt that others perceived her as slow, ugly and a “thought”. Ms. Broadnax feels that she does not see any value in herself or her surroundings. Ms. Broadnax states that she feels down and helpless and that she feels no one is trying to help her. Ms. Broadnax accepted the verbal support and feedback from QMHPA with respect to identifying how to decrease her stress and what parts she plays in her levels of stress. Ms. Broadnax understood that it was important to keep a positive attitude. Ms. Broadnax expressed that stress has affected her sleeping, eating, mood, and her relationship with family and those she is in contact with. Ms. Broadnax explained that stress triggers her mood. Ms. Broadnax shared that when she is depressed, she wishes to be alone and isolated from others. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
